25-year-old man dies after crashing vehicle into tree while fleeing from police

MILWAUKEE — A 25-year-old man died at the hospital Sunday night, November 5th after crashing into a tree while fleeing from Milwaukee police.

The pursuit began around 8:10 p.m. after Milwaukee police attempted to perform a traffic stop on a vehicle near 12th Street and Garfield Avenue. This, after officers suspected the occupant of selling drugs.

Police say the driver fled the scene and crashed into a tree in the area of 10th Street and Lindsay Street.

The driver, a 25-year-old man, attempted to flee on foot but was taken into custody a short time later. He was transported to a local hospital by an ambulance after complaining of pain and was pronounced dead around 10:50 p.m.

Based on preliminary information, investigators believe the deceased may have ingested drugs during the incident.

The Milwaukee County Investigative Team is conducting the investigation with West Allis Police as the lead agency.


1 dead, 2 injured following head-on crash in Jefferson County

JEFFERSON COUNTY — One person is dead and two others were injured in a two-vehicle crash in Jefferson County Tuesday afternoon, November 28th. It happened around 3:00 p.m. on State Highway 16 near County Highway F in the Town of Ixonia.

According to the Jefferson County Sheriff’s Office, initial investigation shows that a car traveling westbound on State Highway 16 crossed the center — striking a pickup truck head on.

The driver of the car was pronounced dead at the scene. He has been identified as 75-year-old Alvin Wolter of Oconomowoc.

The front-seat passenger of the car and the driver of the pickup truck were transported by EMS to Summit Aurora Hospital. The front-seat passenger in the pickup truck was not transported.

The crash remains under investigation by members of the Jefferson County Sheriff’s Office Major Accident Investigation Team.

Driver dies after vehicle strikes tree, catches fire in Waukesha County

WAUKESHA — One person is dead following a crash and vehicle fire early Wednesday morning, December 20th in Waukesha County.

Crews responded to a call of a vehicle fire on Ryan Road, north of Bayberry Drive in the Town of Lisbon around 2:20 a.m.

The first arriving deputy found a car fully engulfed against a tree off the west side of the road.  The Fire Department arrived a short time later and extinguished the fire.

One person, yet to be identified, was found deceased inside the car.

From the initial investigation, it appears the vehicle was traveling southbound on Ryan Raod and left the roadway near the left curve of the road.  The vehicle continued straight, struck a large tree at the edge of the tree line, and started on fire.

There is no evidence of anyone else being in the car.

This crash remains under investigation.

Milwaukee man dies in rollover crash on I-43 near Silver Spring Drive

MILWAUKEE — A 28-year-old man from Milwaukee died early Monday morning, January 1st following a rollover crash on I-43 near Silver Spring Drive. It happened around 1:50 a.m.

According to the Milwaukee County Sheriff’s Office, a driver traveling northbound saw the vehicle hit the median wall, cross all lanes of traffic and then roll over as it went off the road 30 feet up an embankment and into the trees.

An off-duty Greenfield police officer also stopped when he spotted the vehicle off the road and began CPR until deputies, Glendale police and North Shore Fire Department paramedics arrived to continue resuscitation efforts.

The North Shore Fire Department paramedic unit pronounced the man deceased at 2:19 a.m.

Sheriff’s detectives responded to the scene for the crash investigation and notification to the victim’s family.

The man was identified as Jose Ortiz-Hernandez, age 28, of Milwaukee.  He was the lone occupant in the vehicle, and was not wearing a seatbelt.

The medical examiner investigator responded to the scene and gave the official time of death of 3:30 a.m.

The southbound lanes were closed for the investigation and reopened at 6:00 a.m.

1 dead, 2 seriously injured in head-on crash in Kenosha County, driver charged with OWI

KENOSHA COUNTY — A 38-year-old Kenosha County man has been charged with operating a motor vehicle while intoxicated following a fatal crash on Monday, January 1st.

The two-vehicle crash happened around 6:30 p.m. on Highway K in Kenosha County.

Investigation at the scene revealed that a 2006 Toyota Corolla was traveling eastbound and was struck head-on by a westbound 2012 Toyota Tundra pick-up truck.

The operator of the Toyota Corolla, 53-year-old Djuana Latshaw, was pronounced dead at the scene. There was a seven-year-old male passenger in the vehicle. He has seriously injured but is expected to survive. They both are from City of Kenosha.

The operator of the pick-up truck, a 38-year-old man, also sustained serious injuries but is expected to survive.

The investigation revealed that the pick-up truck operator left his lane and entered the lane occupied by the car, causing the crash.

The male operator of the pick-up was alone in the vehicle. He is a Kenosha County resident.

The Kenosha County Sheriff’s Department says the man is charged with operating a motor vehicle while intoxicated, first offense causing death, as well as numerous traffic related charges.

The investigation is still active.

Police: 1 dead following crash near 76th and Dean Road in Milwaukee

MILWAUKEE — Milwaukee police are investigating a fatal crash that happened early Friday morning, January 5th on the city’s north side.

It happened just before 6:00 a.m. near 76th and Dean Road.

Police say a 38-year-old man was driving a vehicle when it left the roadway and struck a tree.  He was pronounced dead at the scene.

MPD continues to investigate the cause of crash.

No additional details have been released — including the cause of the crash or the identity of the victim.

MILWAUKEE — Milwaukee police are investigating a fatal crash that happened early Sunday morning, Jan. 21 near 23rd and Center. It happened around 2:25 a.m.

According to police, a man was driving an SUV westbound on Center Street at a high rate of speed when he lost control and collided with a concrete pole.

The driver died at the scene.

At this time, it appears the Honda was the only vehicle involved and no other persons were injured.

The Milwaukee County Medical Examiner’s Officer is working on identifying the male driver and MPD’s investigation is ongoing.

Driver dies after striking electrical pole, house while fleeing from Racine police

RACINE — One person is dead after police say he crashed his vehicle while fleeing from officers in Racine. It happened around 7:45 p.m. on Monday, January 22nd.

According to police, officers were called out to a home on Yout Street near Green Street for reports of a disturbance. Officers later discovered that the disturbance was an argument between a boyfriend and girlfriend — and the boyfriend was allegedly breaking items in the home.

When police arrived on scene, the boyfriend left the scene in a vehicle at a high rate of speed. Racine police attempted to follow the vehicle, but were unsuccessful.

Officials later discovered an electric pole struck on Charles Street. A stop sign and fire hydrant were also struck. The vehicle eventually stopped after striking a home on Jones Street.

The male driver was transported to Froedtert Hospital — where he later died as a result of his injuries.

The incident remains under investigation.

‘Failed to stop for a posted stop sign:’ 1 dead following crash in Kenosha County

KENOSHA COUNTY — One person is dead following a two-vehicle crash that happened Tuesday night, Jan. 30 in Kenosha County. It happened around 6 p.m. at the intersection of Highway D (184th Ave.) and Highway 50.

According to the Kenosha Co. Sheriff’s Department, the investigation revealed that a 2005 Honda Accord, operated by a 75-year-old woman from Bristol, identified as Charlotte Schroeder, was traveling south on Highway D, and failed to stop for a posted stop sign at Highway 50.

Traveling west on Highway 50 at the same time was a 2005 Acura TL, operated by a 49-year-old man from Salem Lakes.

Authorities say the Honda Accord entered the intersection and was struck by the Acura on the driver’s side. The operator of the Honda was transported to a hospital where she was pronounced dead.

The male was also transported to an area hospital and treated for a minor injury.

An investigation revealed no signs of impairment, and the investigation is still ongoing.

A portion of the intersection was closed for approximately two hours for the investigation.

Heavy fog a factor in fatal crash involving up to 29 vehicles in Verona

VERONA — Authorities say the number of vehicles involved in a fatal crash in the fog on a southern Wisconsin highway is now up to 29.

Dane County sheriff's spokeswoman Elise Schaffer says one person — a 53-year-old woman — died and six others were injured in the pileup on eastbound Highway 18/151 just south of Verona Monday morning, Feb. 9. One of the six injured was transported to the hospital by helicopter. Sheriff's officials say poor visibility due to heavy patches of fog is believed to be a contributing factor.

The sheriff's office says uninjured drivers and passengers were taken to the Verona Fire Department.

Fatal wreck: Tow truck driver suffers medical issue, crashes into Pewaukee home

PEWAUKEE -- A tow truck crashed into a home along Wisconsin Ave. in Pewaukee late Monday, Feb. 12, killing the driver. It's believed a medical issue led to the crash and caused his death.

The driver, who hasn't been identified, was towing a vehicle when something went horribly wrong.

"I heard just a really, really loud bang, kind of a metal on metal kind of thing," said Charlie Smart, neighbor.

The "loud bang" was the sound of the tow truck barreling into the home.

"It was right as the train was passing so I wasn't quite sure what was going on. Kind of scared me a bit," said Smart.

Police said the tow truck driver with Ken Weber Towing picked up a vehicle shortly before the crash. Police say he lost control of the truck.

"I seen the truck sideways against the building. Roads are all blocked off. There were a bunch of firefighters," said Don Lloyd, neighbor.

The driver went through a couple of yards before crashing.

Neighbors said first responders tried to cut the tow truck open to get the man out. He was pronounced dead at the scene.

"A very sad, unfortunate event," said Lloyd.

Police said it appears the driver died from a medical condition, and not from the crash.

There was at least one person inside the home at the time. They were not hurt.

"Those are my brothers. We work on the house and we know her personally," said Lloyd.

Neighbors helped the homeowner Tuesday by boarding up the front window.

"To keep the heat in the house," Lloyd said.

While the home was damaged, firefighters said they believed it to be structurally sound.

The crash remains under investigation by the Wisconsin State Patrol.

Juan Mata-Chavez, 27, pleads not guilty in hit-and-run crash that killed mother, son

MILWAUKEE — A man prosecutors say tried to have his sister’s pickup truck reported as stolen at gunpoint after a violent hit-and-run crash caught on camera, which took the life of a mother and injured her son after they left church on Super Bowl Sunday was in court Wednesday, Feb. 21.

Juan Mata-Chavez, 27, waived his preliminary hearing. He then pleaded not guilty. A scheduling conference was set for March 5.

Four people fled the truck after the crash, and one of those people, a passenger in the truck at the time, helped police identify the driver — Mata-Chavez. Prosecutors say he has a revoked license due to a prior OWI offense.

Mata-Chavez turned himself in to police on Feb. 8.

The crash happened Sunday, Feb. 4 near 15th and Lincoln, and it was caught on camera.

Monica Hernandez, 43, died as a result of injuries suffered in the crash. Her son, 13, was hurt. They were coming back from church at around 11 a.m. when it happened.

According to a criminal complaint, a Toyota Corolla was found at the scene with severe damage to the front end. Aid was being rendered to Hernandez. At the hospital, it was determined she had suffered broken femurs and “avulsions” in both legs, along with internal bleeding. She died during surgery, and an autopsy revealed the cause of death as multiple blunt force injuries secondary to a motor vehicle collision.

Her son told investigators he was the front seat passenger of the Corolla, driven by his mother. He said their vehicle was struck by a blue pickup truck.

A witness told investigators he was inside the nearby La Conoa restaurant when he heard the loud crash outside. He said he saw a Toyota had struck his vehicle outside the restaurant, and there was a blue Ram pickup truck in the middle of the intersection. He said he saw three people — two males and a female, get out of the truck through the driver’s side door. He said he yelled at them not to leave. He said one of the males, wearing a red shirt, pointed a handgun at him — so he ran to his own car to get his own gun. He said he then realized all three people had left the scene.

“He told them ‘please don’t leave. Why are you leaving? Face the consequences.’ They pulled a gun out and put it in his face,” Nattalie Lopez, witness said.

Another witness who went to the truck after the crash to see if anyone needed help was able to provide a description of the individuals inside. She said the person in the driver’s seat was a Hispanic male wearing a blue baseball cap and the person in the front seat was a Hispanic male wearing a red shirt. The person in the back seat was a Hispanic female wearing a white sweater. When she saw the  man in the red shirt holding a gun, she went back into the restaurant.

Surveillance video shows a pickup truck headed east on W. Lincoln Ave. at a high rate of speed, before it spins out of control, crosses the center line and collides with the Toyota.

Several witnesses rushed to try to help the mother and her son.

“They tried to help the mom and she was screaming ‘where’s my son? Is he OK?’ She was very bad. Her leg was broken,” Lopez said.

Lopez tended to the child.

“He was very sad. I noticed his hand was bleeding. I could only imagine the pain he was going through right now. I didn’t want him to see his mom like that,” Lopez said.

“The look in her eyes was hopeless. Her face is severely injured. She is trying to ask for help. The look on people’s face — they are helpless and you cant hep them. It haunts you,” Loai Atali said.

Investigators checked DOT records, which revealed the owner of the truck. That truck was reported stolen to Milwaukee police on Feb. 4 — about a half-hour after the crash happened. The owner reported it was taken in an armed robbery — but during questioning, the owner admitted that was false, and her brother, Juan Mata-Chavez, had her truck. She said he called her and told her to report the truck had been taken in an armed robbery. She identified her truck as being involved in the crash after she was shown surveillance video.

Milwaukee police received information that helped them identify the female in the truck at the time of the crash. She came in to police and reported she was inside the vehicle at the time of the crash, and she heard the female in the other car had died. She said “she could not live with that in her heart.” The complaint says she told investigators she got a message from her friend “Carlos Mata” asking if she wanted to get something to eat. She agreed, and he picked her up around 10:30, and there were two other males in the car — with “Carlos” driving. She said they were headed east on Lincoln at a normal speed, and when she saw a man crossing the street, “Carlos” braked, and lost control of the truck, which went sideways. She said she saw a car coming toward them but she didn’t remember the impact. She said the next thing she remembers is someone opening her door, grabbing her hand and running away from the crash scene. She said she and the three males eventually split up and she called her sister for help.

She was able to identify the driver of the truck as Juan Mata-Chavez, a person she knew as “Carlos Mata.”

Police say records show Mata-Chavez had been issued a probationary license which had been revoked on May 13 following a municipal citation for OWI.

The medical examiner’s report from this incident indicates the pickup truck was being driven “at freeway speeds on a city street,” with snow and slush on the street. The posted speed limit in the area is 30 miles-per-hour.

Mother arraigned on DUI, manslaughter charges in June 2017 crash that killed daughter, 8

SCRANTON, Pa. -- A Pennsylvania mother is facing DUI and manslaughter charges in connection with a June 2017 crash that killed her daughter, 8.

Police say Linette Villa of Scranton, Pa. had drugs in her system, and the little girl was not properly buckled in the car. Villa was taken to the Lackawanna County Prison after being arraigned on charges of DUI and involuntary manslaughter Tuesday, March 6.

Her daughter, 8, was killed in the June crash and another daughter, 6 was badly hurt.

Villa didn't have anything to say to WNEP's cameras during her arraignment Tuesday, but wept as a magistrate went through the charges against her, including involuntary manslaughter and DUI for the crash on the North Scranton Expressway.

According to court documents, other drivers saw Villa's blue Honda speed past them before losing control and hitting a concrete barrier.

An autopsy found that Andrea Morales, 8,  was killed instantly. Her sister was flown to a hospital, but has since recovered.

Now, police say their mom had cocaine and marijuana in her system at the time of the crash. A reconstruction of the crash showed that neither girl was properly buckled. They were only wearing lap belts.

Villa is facing even more charges because of what happened when police went to arrest her Tuesday afternoon. Police say Villa fought them and had cocaine on her. She's locked up on $225,000 bail.

10 hockey players, 5 others dead as a result of fatal bus crash, police say, $4.5M+ raised for families

CANADA — More details about the victims of a deadly bus crash in Canada’s Saskatchewan province emerged Sunday, days after the tragic accident rocked their small community and the larger hockey world.

The bus was carrying a junior hockey team when it collided with a tractor-trailer Friday evening, according to police. Fifteen people were killed and at least 14 others injured.

On Sunday evening, the Royal Canadian Mounted Police and the Office of the Chief Coroner released the full list of those who perished in the crash. Ten Humboldt Broncos players and 5 others, including two coaches, died, the release said.

Among the dead was Jaxon Joseph, the 20-year-old son of former NHL player Chris Joseph.

Chris Joseph used to play for the Pittsburgh Penguins, and the team expressed sadness about the news on Twitter. “We offer our heartfelt condolences to Pittsburgh Penguins alumnus Chris Joseph, whose son, Jaxon, was among those who tragically passed away in the Humboldt Broncos bus accident.”

Jaxon’s former teammates on the Surrey Eagles told CNN broadcast partner CBC that he’d be missed.

“He was just a hardworking guy,” said Jackson Ross. “He always had a smile on his face. It was contagious, I’d say.”

Important to know the victims, brother says

Player Xavier Labelle, also died, his brother told CNN in an interview Sunday. “Xavier was a great person and nobody ever said anything bad about him,” Isaac Labelle said, adding that his brother was “well-loved by everybody.”

Both of the brothers grew up playing piano and sports together, Isaac said. He and Xavier, 18, were best friends.

“I also want people to know the good person he is,” Isaac said. “As much as I know it’s hard for me to talk about it, it’s important for people to know the victims of this tragedy.”

Logan Schatz, a 20-year-old team captain and Saskatchewan native, was also killed.

His father, Kelly Schatz, told the Canadian Press family members were comforting each other during this difficult time.

A website for the Saskatchewan Junior Hockey League said Schatz had been named player of the month in February.

The other players who died were Adam Herold, 16; Connor Lukan, 21; Evan Thomas, 18; Jacob Leicht, 19; Logan Boulet, 21; Logan Hunter, 18; and Stephen Wack, 21, the RCMP said.

‘Incredible’ head coach among the dead

The team’s head coach, Darcy Haugan, 42, died in Friday’s crash, according to the release.

When asked about Haugan in a press conference Sunday, Broncos President Kevin Garinger paused and held back tears, before telling reporters Haugan was an “incredible” coach, husband and father.

“He talked about the fact that the Humboldt Broncos were about not building hockey players, but creating amazing young men … and that was Darcy’s belief, and through that process they would’ve ultimately become great hockey players, too, which was also the case.”

“He’s sadly missed by our entire organization,” Garinger said.

The driver of the bus, 59-year-old Glen Doerksen, died in the crash.

Another team Doerksen drove this spring, the Kinistino Tigers Senior Hockey Team, said on its Facebook page that Doerksen enjoyed being able to transport hockey teams and watch them play their games.

“We got on that bus and trusted him with our lives, and both times he got us home safely,” the post said.

Assistant coach Mark Cross, 27, also died.

Broadcasting employees killed in crash

Two employees of local radio station Bolt FM who were traveling with the team also lost their lives.

Both the radio station and the website DiscoverHumboldt.com belong to parent company Golden West Broadcasting.

The site confirmed that play-by-play announcer Tyler Bieber and Brody Hinz were killed.

“It is with great sadness that we have learned both Tyler and Brody were victims of this tragedy,” said Lyndon Friesen, the president of Golden West Radio, a subsidiary of Golden West Broadcasting, in a statement. Bieber traveled with the team often, and Hinz had just recently joined the company.

Cause of the crash is being investigated

The crash occurred around 5 p.m. Friday north of Tisdale, according to the Royal Canadian Mounted Police, as the team was headed for the town of Nipawin for a playoff game. RCMP Assistant Commissioner Curtis Zablocki said the tractor-trailer, traveling west on Highway 335, collided with the bus, which was traveling north on Highway 35.

Three of the injured are in critical condition, according to the RCMP. Authorities have yet to identify the victims. Twenty-nine people were on the bus.

The driver of the tractor-trailer was not injured in the collision, Zablocki said. He was initially detained but was later released, and he is not currently the subject of a criminal investigation.

“Our investigators will be looking at all aspects of this accident to determine what took place,” Zablocki said. “It’s too early to comment on the cause of the collision.”

The Humboldt team was scheduled to play the Nipawin Hawks in the fifth game of a best-of-seven semifinal playoff series Friday. The teams played a lengthy game Wednesday that Nipawin won 6-5 in three overtimes in Humboldt, leaving Nipawin with a 3-1 series lead.

Pain felt far and wide

The tragedy was felt across Canada, where hockey is the dominant sport and many young players take long bus rides to tournaments.

Humboldt, a town of about 6,000, has a long tradition of fielding teams in the Saskatchewan Junior Hockey League. The players, ranging in age from 16 to 21, come from different parts of Canada and stay with host families during the season, according to the team’s website.

“Our Broncos family is in shock as we try to come to grips with our incredible loss,” Garinger, the team’s president, said in a statement.

The hockey community and others rallied around the victims and their families after news of the crash, offering financial assistance.

By Sunday evening, a GoFundMe page for the players and their families had raised more than $4 million, the result of nearly 60,000 donations.

Many of those donations purportedly came from the hockey community. NHL teams like the Toronto Maple Leafs and the Pittsburgh Penguins gave money, as did hockey equipment manufacturer Bauer Hockey, which shared a link to the GoFundMe on its Twitter account.

Expressions of support poured in, much of them focused on a Twitter photo that showed three injured players grasping each other’s hands while lying in hospital beds.

Members of the National Hockey League expressed sadness over the news.

“We sent condolence, comfort and strength to all affected by the devastating crash involving the Humboldt Broncos’ team bus,” the National Hockey League said on Twitter.

Cam Talbot, a goalie for the Edmonton Oilers in the NHL, tweeted: “After all the hours spent on a bus with the boys over the years this one really hits home. Our thoughts and prayers go out to the families and communities affected by this horrible tragedy. #PrayersForHumboldt”

Friday’s crash brought back memories of another highway accident that claimed the lives of hockey players in Saskatchewan. In 1986, four members of the Swift Current Broncos of the Western Hockey League died in a crash in the western part of the province, CNN partner CBC reported.

KENOSHA — A 47-year-old woman from Illinois died Saturday, April 28 following a crash involving multiple motorcycles in Kenosha County. It happened on Highway W around 6:30 p.m.

According to the Kenosha County Sheriff’s Department, the investigation into this crash revealed that a group of motorcyclists from Illinois were traveling southbound when the lead operator of a 2006 Harley-Davidson motorcycle failed to negotiate a curve in the road and lost control of his motorcycle.

This motorcycle slid into another motorcycle, a 2010 Harley-Davidson, which had an operator and a passenger. This created a domino effect and another motorcycle, a 1998 Kawasaki,  also crashed.

The passenger from the 2010 Harley-Davidson motorcycle suffered serious injuries.

Flight for Life was called to the scene — and landed in a nearby parking lot.

The female passenger, identified as 47-year-old Lisa Fagan from Illinois, was pronounced deceased at the scene.

There were five other injuries reported in this crash ranging from minor to significant.

The roadway was closed for authorities to properly investigate the scene.

At this time speed appears to have contributed to this crash.


Best friends killed in crash after driver lost control, hit utility pole in Cedarburg

OZAUKEE COUNTY -- Two families are grieving after a single-vehicle crash in the Town of Cedarburg Monday night, June 25. Two men, ages 21 were killed -- and we're told they were best friends.

"He would go on rides for hours," said Brooke Krumbiegel, Devin Krumbiegel's sister.

On Monday evening, their drive turned fatal.

"He's touched so many lives and he will be missed and always in so many people's hearts," said Brooke Krumbiegel.

Around 6:30 p.m., officials with the Ozaukee County Sheriff's Office said Riechers was behind the wheel of a Honda Civic with Krumbiegel in the passenger seat, when he lost control on Western Avenue near Woodside. The car entered a ditch, striking trees and a utility pole. Friends and family said the two died on impact.

Fighting back tears, Brooke Krumbiegel said while her brother may have been the youngest of four siblings, she always looked up to him.

"He was the strongest person I knew. He was always there to help anyone in need. He would drop anything to help any of his family members," said Brooke Krumbiegel

At Riechers' house, childhood friends placed flowers and photos on his bike -- remembering his signature smirk and great sense of humor.

Both sets of loved ones are organizing a ride for July 8, to raise money for funeral costs and give the young men the goodbye they would have wanted.

Sheriff's officials said speed appears to be a factor in the crash. Alcohol and drugs are not suspected.

MILWAUKEE — Milwaukee police are investigating a fatal crash that happened Monday, Jan. 7 near 35th and Townsend. It happened around 11:15 a.m.

According to police, a vehicle driven by a 61-year-old Milwaukee man struck a parked vehicle and despite life-saving efforts, the man died on scene.

MPD is still investigating the circumstances surrounding the crash.

DODGE COUNTY —  One person is dead and another was flown by Flight for Life to Froedtert Hospital following a crash late Thursday night, Jan. 10 on I-41 near State Highway 28 in Dodge County.

According to the Dodge County Sheriff’s Office, at approximately 11:59 p.m. officials were dispatched to a two-vehicle crash on Interstate 41 near STH 28 in the Township of Lomira.

The preliminary investigation shows a silver Ford pickup truck was northbound in the southbound lanes of I-41 when it struck a southbound Peterbilt Semi that was in the right lane.

The driver of the Ford pickup was pronounced dead at the scene by the Dodge County Medical Examiner.  The name of the driver is being withheld pending notification of next of kin.  There were no other occupants in the vehicle.

The Peterbilt driver was injured in the crash, sustained non-life threatening injuries and was flown by Flight for Life to Froedtert Hospital.  The name of the driver is being withheld at this time.

The southbound lanes of I-41 were closed for approximately five hours and 20 minutes — but are now now.  The northbound lanes of I-41 were closed for approximately one hour and ten minutes while Flight for Life was on the ground.

The crash remains under investigation by the Dodge County Sheriff’s Office and the Dodge County Crash Investigation Team.

Assisting at the scene was Dodge County Medical Examiner, Dodge County Highway Department, Wisconsin State Patrol, Washington County Sheriff’s Office, DCERT, Lomira Fire Department, Theresa Fire Department, Theresa EMS, Fond Du Lac Paramedics, and Flight for Life.

WASHINGTON COUNTY -- Two men died and three children were injured in a head-on crash in Washington County Sunday night, March 17.

It happened around 7 p.m. in the area of County Highway NN, east of County Highway M in Jackson.

"The first vehicle came into the second vehicle's lane, and that's how they struck," said Jackson Fire Chief Aaron Swaney.

According to the Washington County Sheriff's Office, a preliminary investigation revealed the first vehicle was headed eastbound on County Highway NN when it veered into the westbound lanes -- striking the second vehicle head-on.

Both drivers died.

"We don't really see too many fatal accidents on back roads, or in our community. Most of our accidents are on freeways," said Chief Swaney.

The operator of the first vehicle was identified as a 52-year-old man from the Town of Barton. The operator of the second vehicle was identified as a 51-year-old man from West Bend.

There were three passengers in the second vehicle, ages 8, 10 and 16. They were all transported to a hospital with injuries not believed to be life-threatening.

Speed was believed to be a factor in the crash, officials said.

"Slow down. Take precautions at all times. It's very easy to get into an accident. Even if you are an excellent driver, it still can happen," said Chief Swaney.

"All I heard was a big crash... go boom," said Moore. "Come outside my house. There's a body outside my house."

Officials say 19-year-old McCoy was killed after the suspect's vehicle collided with his near the intersection.

"I've never seen anything like that before," Moore said.

The lengthy chase began miles away, near 64th and West Congress. Officers were pursuing a reckless, stolen car which disregarded multiple traffic signs.

"Stealing cars and stuff, they need to stop that," said Moore. "An innocent person just lost their life for some stuff he had nothing to do with."

The suspect survived. Officers and MFD attempted life-saving measures on McCoy, however, he died at the scene.

"I have never seen anyone do CPR as long as they were doing on that man. Trying to bring him back," said Moore.

Police are now investigating the deadly chase and crash as a family mourns a lost loved one.

The driver of the stolen vehicle is a 19-year-old Milwaukee man who suffered a serious injury. He was taken to a local hospital and is in custody.

WILMINGTON, NC (WWAY) -- Investigators say Friday evening two girls ran a stop sign while riding a child's toy motorbike. Now those nearby tell us what they heard and saw that night.

Witnesses, who did not want to go on camera, say Friday around 6 p.m. they heard a loud bang, followed by screams and several sirens near 8th and Campbell Streets, and they rushed outside to see what happened.

"I heard a loud boom,” neighbor Terrell Askew said. “It sounded like something hit something. But I thought it was them, and them playing so I paid no attention until I heard all the fire trucks and all the people screaming."

"Just a loud smack, metal on metal,” another neighbor, Mark Bryant explained. “And immediately when you heard it you knew that something had just happened that probably was going to be not good."

When they stepped outside to see what the noise was all about, they were met with a frightening sight.

"Horrific,” Bryant said. “I mean if you see legs hanging out from under a car and a frantic driver, obviously that's something that raises concern."

Investigators say two girls, a 2-year-old and a 14-year-old, ran a stop sign while riding a pink child's toy motorbike.

Bryant says it only took responders minutes to arrive, but getting the trapped teen out took responders extra time.

"It took less than 20 minutes for them to lift the vehicle and get the 14-year-old girl out,” Bryant explained. “They were able to get the small child out immediately."

Both girls were taken to the hospital. There is no word yet on their conditions.

Parents who live nearby say they will be keeping a closer eye on their kids.

"I have to walk across the street with a grown-up,” Ravyion Cook said about his parent’s new rules.

And kids say this has taught them some very important lessons.

"Everybody should be careful when they cross the street,” D-Cory Shellman said.

There is no word yet if charges will be filed against the driver.

Those who live near the where the accident happened are asking people to slow down, and pay attention.

UPDATE: Marine identified as pedestrian killed in weekend crash

WILMINGTON, NC (WWAY) -- Wilmington police are investigating after Marine Cory Lee Reese, 22, was hit and killed in the 200 block of Rose Avenue early Sunday morning.

Police say the driver, Jackson Ruelle Davies, 19, was heading south toward Wrightsville Avenue when he hit Reese before running off the road and slamming into a tree and utility pole.

Ben Jones lives just down the road from where the accident happened. He says when he came home from work he was met with a frightening sight.

"There were about 30-40 cop cars out here as well as ambulance and fire department," Jones said. "I believe that he was trapped in the car for a while, and it looked like they pulled him out and put him on a stretcher going to an ambulance."

Davies had to be extricated from the car and was taken to New Hanover Regional Medical Center. He is charged with DWI, and police say additional charges are pending.

Reese was later pronounced dead.

Man who slammed car into home charged with DWI, reckless driving

WILMINGTON, NC (WWAY) -- A man who police say slammed his car into a Wilmington home early Saturday now faces charges for DWI and reckless driving.

The crash happened shortly after midnight Saturday at the home at 6712 Yarmouth Way. Police say when they got to the scene, they discovered a black Ford Mustang had smashed into the back of the home where people were inside.

Police say the Mustang went airborne before crashing into two fences, then hitting the home, and finally landing in a neighbor's yard after hitting another fence.

The driver, Adam Matthew Battersea Molto, was taken to the hospital for minor injuries, along with the passenger in his car. Molto is charged with DWI and careless and reckless driving.

Police say the home has been hit once before.

Holden Beach, NC (WWAY)-- Sunday morning around 7:45, the Brunswick County Emergency Communications received a report of a motor vehicle crash involving a bicyclist.

This happened at the intersection of Holden Beach Road and Ocean Blvd West.
Crews found that a passenger car turned into the path of the bicyclist.
The bicyclist then hit the car.

The Chief of Police, Wallace Layne, says the bicyclist was transported to New Hanover Regional Medical Center to treat injuries.
The driver of the car was cited for failing to yield to on coming traffic.
The case is still under investigations. Names are not being released at this time.

Pender Co. twin takes to Twitter to bring Drake to sister's funeral

PENDER COUNTY, NC (WWAY) -- A former Laney High School cheerleader is at the center of the Twitter-verse, as family members work to honor her memory.

Jasmin Grady died early Sunday morning in a car accident on Highway 17. Now, her twin brother is working to fulfill her last wish, which has Twitter trending in a big way.

Kenneth Grady, 20, said he and his twin sister Jasmin used to talk about what their last wish would be. He said it all started with a conversation with his sister.

"Just random little conversations," Kenneth said. "She said if she was to ever pass, she wanted a big funeral, and she wanted Drake to sing at her funeral."

So that's what Kenneth set out to make happen. The night after she passed away in a car accident, he took to Twitter, and Twitter took off.

In a matter of hours, Kenneth's hashtag topped the U.S. trending chart on Twitter, with tweets pouring in from all over. It even gained recognition from a few celebrities, like Keke Palmer and Montel Williams.

Former cheerleading teammate Natia Simmons said she kept up with Jasmin through social media after high school. "Her tweets, her posts on Instagram, her vines, everything," she said. "We're really going to miss seeing her."

Getting Drake to come to the Port City might seem like a tall order, but Kenneth said even if he doesn't come, that would be OK.

"I'm just glad for all the support she's receiving, and my family is receiving," Kenneth said.

Jasmin was going to be a junior at North Carolina A&T this fall.

UPDATE: Driver taken to hospital after car crashes into strip mall

WILMINGTON, NC (WWAY) -- Police say the driver of a car that crashed into a building Thursday night was taken to the hospital after they say he lost control because of a medical condition.

The crash happened around 8 p.m. off of Eastwood Road at Racine Drive, causing some damage to a Wilmington strip mall there. Police say the driver lost control, sending the car into a pillar right in front of Pro Golf.

Wilmington Police say the driver has a medical condition, which likely caused the crash. They say the driver was taken to the hospital for injuries, but is expected to be OK.

No word yet on whether or not charges will be filed in the crash, according to police.

The accident happened at the corner of Kerr and Martin Luther King.

Wilmington Police and Fire Departments responded to the scene.

No one was hurt in the crash. However, fire crews said there is some structural damage to the home.

UHP responds to over 150 weather-related car accidents in one day, including 16-car pileup

SALT LAKE CITY — A 16-car pileup brought traffic to a complete stop on eastbound SR-201 near 3200 West Thursday evening.

According to Sergeant Nick Street with Utah Highway Patrol, the pileup consisted of a four-car crash, two three-car crashes and three separate two-car crashes. They all occurred at the same time, in the same stretch of 201.

Street explained the icy roads were much slicker than anticipated, and that many cars were going too fast for conditions.

Drivers involved in the crash reported slowing down, but said the roads were so icy that it was difficult to keep control. Once one crash happened, they indicated it created a domino effect on everyone else.

"People were just sliding all over the place," said Ryan Johansen, who was involved in the pileup. "People just couldn't stop. I couldn't stop, either, even though I had my foot all the way on my brakes."

Marie Antoine said she had just gotten on 201 at Bangerter, and wasn't going very fast. She slowed down upon seeing the crashes, and said a car in front of her slid. She couldn't get over in time, and said her vehicle couldn't stop.

"I end up hitting her. And then about 10 seconds after the accident, someone slammed into my left side," Antoine recounted.

Nathan Castagno was able to stop without hitting anyone or being hit, but he still couldn't escape the pileup.

"Got stopped, just in time," he said. "Then next thing you know, I got hit in the rear."

Although there were some injuries, Sgt. Street said they were all minor and treated at the scene.

UHP investigated more than 150 crashes on Thursday, including multiple slide offs and crashes on highways throughout the state. One car nearly ended up in a pond off of I-80, after troopers said a lane change in the slush sent the vehicle toward the water near the Saltair.

"Sometimes the roads may not look like they're slick, but it's been snowing all day. The temperatures are at freezing," said Trooper Steve Meyer, at the scene of that slide-off.

Later Thursday evening, a Draper City Fire battalion chief was hit by a car while responding to a crash.

UHP urged drivers to slow down and leave extra space in front of the vehicle.

Draper Battalion Chief ‘doing well’ after being hit by car on I-15

MURRAY — A Battalion Chief for the Draper City Fire Department is expected to make a full recovery after he was hit by a car on southbound I-15 Thursday evening.

"We are so relieved to let you know that our Battalion Chief who was struck by a car last evening on I-15 is doing well. He does not have any broken bones and is recuperating in the hospital," a Facebook post from the Draper City Fire Department said.

Battalion Chief Bart Vawdrey was flown to Intermountain Medical Center after he was struck, but Draper City officials said he didn't suffer any broken bones or internal injuries.

"He’s doing really well," said Draper City spokeswoman Maridene Alexander late Thursday night. "We are just elated to report that he’s doing really great."

Vawdrey was initially reported to be in critical condition.

Utah Highway Patrol Sergeant Nick Street explained that  Vawdrey had responded to a two-vehicle crash between 14300 and 14600 South on southbound I-15.

One vehicle had pulled over to the right shoulder, and the other to the left shoulder.

While a UHP trooper helped the vehicle on the right shoulder, Street said, Vawdrey pulled over to help the driver of the vehicle on the left shoulder.

When Vawdrey got out of his SUV, another car going too fast in the icy conditions lost control and hit him. The car spun around backward and pinned Vawdrey to the bumper of the car he was trying to assist, Street explained.

According to Alexander, Vawdrey saw the vehicle lose control and he was able to jump partially out of the way.

"He’s a tough guy. So if anyone would survive this, it would be him," Alexander said. "He’s just that kind of guy, so I’m just-- it didn’t surprise me when I heard that he was able to kind of escape any terrible injury this evening."

She said Vawdrey will stay overnight in the ER at IMC in Murray, so doctors can monitor him. Alexander indicated Vawdrey's family is grateful he will be coming home and making a full recovery.

"We just are so happy to report he’s doing so well. He’s talking, and he’s a tough guy. He really is," she said. "So, we’re just pleased to see he’s doing so well."

UHP troopers responded to more than 150 crashes Thursday, including a 16-car pileup on SR-201.

Street warned drivers to slow down and leave extra space in front of them. UHP and UDOT indicated that the icy conditions will persist in the coming days.

Last night Battalion Chief Bart Vawdrey was struck by a vehicle on I-15 while tending to victims involved in car accident. Battalion Chief Vawdrey was treated by his own crews from Draper City Fire Department and transported by LifeFlight to Intermountain Medical Center. We are extremely grateful that his injuries are not life threatening and that he will make a full recovery. Battalion Chief Vawdrey’s injuries could have been avoided by simply slowing down for the current weather conditions and paying attention to the surroundings. Firefighters and law enforcement officers put their lives on the line every day for the protection of others. We plead with everyone to please slow down , eliminate distractions, and focus on their surroundings when traveling in adverse weather conditions. Please watch out for our firefighters and police officers that are working on the roadways to keep us all safe and move over to give them the safe area they need to perform their duties. It is a miracle that we are able to talk about awareness today and not what the outcome could have been. Powder Mountain Road closed after accident leaves one dead

WEBER COUNTY — Deputies responded to a traffic accident on Powder Mountain Road Wednesday that left one dead and one injured.

According to a press release, deputies located a cargo truck on Highway 158 that had been traveling down the road on its side in both lanes of travel. A 37-year-old male passenger had been ejected and was pronounced dead on the scene.

The driver sustained minor injuries and was treated on scene.

There were no other occupants in the truck and no other vehicles involved.

Officials investigating the incidents believe brake failure may have been a factor.

Powder Mountain Road is currently closed in both directions and is anticipated to be closed for several hours.

Those who were coming down the road before the closure have been diverted back up to Powder Mountain Ski Resort, and people are advised to remain in the lodge where it is warm until the road is opened.

Police: 81-year-old woman dies after husband tries to pass vehicle while driving in fog

WELLS, NV. — An 81-year-old woman from Canada died Saturday after officials say her husband ran into a truck head-on while trying to pass a car in the fog.

According to a press release made by the Nevada Highway Patrol, at 8:30 a.m. troopers responded to a fatal vehicle crash on US-93, approximately 15 miles north of Wells.

A preliminary investigation indicated that Oscar Vandenbossche, 82, from Columbia Canada, was driving a Toyota Rav 4 southbound on the highway when he attempted to pass a slower vehicle in dense fog, officials said.

While Vandenbossche was trying to pass the vehicle, he collided head-on with a Dodge truck that was heading northbound on US-93. Officials said the truck was pulling a horse trailer with three horses inside.

The truck driver sustained minor injuries, according to investigators, but none of the horses were hurt.

“The right front passenger in the Toyota Rav 4, Mrs. Maryann Vandenbossche, age 81, from Osoyoos, British Columbia, Canada, died at the scene,” police said. “Mr. Vandenbossche was flown to University of Utah where he is in critical condition.  Both occupants were wearing their seatbelts.”

Officials with the highway patrol reminded drivers to use extreme care when driving in inclement weather and conditions.

Highway Patrol Trooper barely dodges out of control driver

WILLARD, Utah — On Wednesday, Trooper Steve Shaffer’s day off, he geared up and hit the cold, slippery Utah highways to help drivers who slid off the road due to snow.

During that morning commute, around 11:00 am, Shaffer said he had already dealt with a number of slide-offs and accidents. One incident he saw dealt with a jackknifed semi and a car rollover on mile marker 306, outside of Perry.

“I was behind the scene and was going to provide traffic control to get people to slow down and move over to provide safety for other troopers and others involved,” Shaffer said.

Shaffer pulled his cruiser off on the shoulder, as close to the cement barrier as he could get it, and started unloading flares so people knew to move over.

“I turned back to open liftgate and I heard somebody sliding and I looked behind me and a car was coming towards me,” Shaffer said.

That car slid and nearly hit Shaffer as he stood at the back of his UHP cruiser.

“I just barely had enough time to get over to the median wall and get over the wall before she collided with the back of my car,” Shaffer said.

When that car came sliding towards him and his car Shaffer said he had images flashing through his mind.

“Many things go through your mind even in the short span. I had images of me getting struck and knowing that people get struck,” Shaffer said. “It can be avoided if people would just leave earlier on a snow day and give themselves plenty of time so they can drive slower.”

One in critical condition, several injured in crash on I-15 in Juab County

JUAB COUNTY, Utah — One man is in critical condition and a woman and her child sustained minor injuries in a crash on northbound Interstate 15 Thursday.

According to the Utah Highway Patrol, a Dodge Dakota truck was traveling north while the road was wet and it was raining. The driver lost control and traveled through the median, onto the southbound lanes, where it struck a Ford Focus.

UHP said both vehicles sustained significant damage.

The male driver of the Ford was wearing a seatbelt but was trapped in his vehicle and officials were unable to get him out right away.

The male driver sustained numerous serious injuries, UHP stated, and was taken to Central Valley Medical Center and then transported to Utah Valley Hospital with internal bleeding. He is in critical condition.

The driver of the Ford Focus, a female, sustained minor injuries. A female passenger broke her arm, and a child in a car seat suffered a collapsed lung. Both were transported to the hospital.

Man arrested for automobile homicide after fatal crash in South Jordan

SOUTH JORDAN, Utah — A fatal rollover crash forced police to close a section of Lake Ave. in South Jordan Wednesday morning.

According to a press release from the South Jordan Police Department, initial reports said a male had been ejected from his vehicle while an unconscious female was pinned inside a vehicle.

The department said a GMC K3500 pickup truck traveling eastbound ran a red light, colliding with a Dodge Caravan traveling northbound on Mountain View Highway.

The pickup truck rolled and its driver, 23-year-old Edilberto Lopez-Ramirez, reportedly crawled out of the vehicle on his own. He was taken to a local hospital with non-life threatening injuries.

The driver of the Dodge Caravan, 38-year-old Pearl Iata of Herriman, was killed in the crash.

The truck Lopez-Ramirez was driving was reported stolen from a job site in Herriman roughly an hour before the crash, the press release stated. The same truck was also involved in a hit and run crash at South Jordan Parkway and southbound Mountain View Highway prior to the crash that took Iata's life.

Police believe alcohol was a factor in the crash that killed Iata, and Lopez-Ramirez is the suspect in both crashes.

According to the press release, Lopez-Ramirez admitted to stealing the truck, drinking beer before the crash, running the red light that took Iata's life and being involved in the earlier hit-and-run.

Lopez-Ramirez has been booked into the Salt Lake County Jail for auto theft, automobile homicide, felony DUI, hit and run, reckless driving, and driving with an invalid driver license.

SALT LAKE CITY — The Salt Lake City Police Department tweeted Wednesday night that they have arrested a suspect in a hit-and-run that occurred Tuesday night.

Police arrested 36-year-old Sotero Valerio Torres and booked him into jail on charges related to a hit-and-run Tuesday night which resulted in the death of Tot Nguyen, who was riding his bike at the time.

Nguyen was transported to the hospital after the incident and later succumbed to his injuries.

 

Police identify 4-year-old killed in South Jordan car crash

SOUTH JORDAN, Utah — A 4-and-a-half-year-old boy died following a car crash Wednesday that left several others injured.

Police identified him as D’mondre Brown and said in a press release he was seated in the front passenger seat of a Dodge Neon without a child restraint or booster seat, although he was wearing a seatbelt.

Following a three-car crash at 10400 South and Bangerter Highway, deployed airbags critically injured D’mondre. He was transported to the hospital in critical condition but shortly died from his injuries.

Lt. Matt Pennington with South Jordan Police said that so far, it looks like the Dodge Neon rear-ended another vehicle that had stopped for traffic backing up in front of it. That vehicle then hit a third vehicle.

According to the press release, the exact cause of the crash is still under investigation.

The area of the crash has since been re-opened. In their press release, South Jordan Police reminded drivers that Utah law states children up to age 8 must be properly restrained in a car seat or a booster seat, and it is recommended that children under the age of 13 ride in the back seat of a vehicle.

This is a developing story. FOX 13 will update it as more details become available.

Minor crash turns ‘tragic’ as improperly restrained 4-year-old killed in South Jordan

SOUTH JORDAN, Utah — A young boy died Wednesday after an airbag deployed during a crash and struck the boy, who was not restrained in a child seat.

Police identified the victim as D’mondre Brown.

They said D’mondre was sitting in the front seat of a Dodge Neon driven by his father, who failed to brake for stopped traffic and rear-ended another car. The impact caused an airbag to deploy.

“That airbag caused catastrophic injuries and the 4-and-a-half-year-old child ended up dying at the hospital from those injuries,” said Lt. Matt Pennington of the South Jordan Police Department.

The crash happened around 4 p.m. Wednesday afternoon on South Jordan Parkway, just east of the Bangerter Highway intersection.

Police spent more than three hours investigating, but they are unsure why the driver didn’t stop. They don’t believe speed was a factor.

“Those airbags will go off in an under a 10 mile an hour crash,” Pennington said.

Police believe this accident had deadly consequences because the child wasn’t properly restrained in a child seat.

“We see accidents like this almost daily, and very rarely is there significant injury,” Pennington said. “What is typically a minor rear-end accident, has turned into a tragic incident.”

Utah law states children up to age 8 must be properly restrained in a car seat or a booster seat, and it is recommended that children under the age of 13 ride in the back seat of a vehicle.

“That airbag is coming out at approximately 200 miles per hour,” Pennington said. “Even though you might be going forward at only 30, that's a 230 mile per hour collision with your body.”

No one else involved in the accident suffered serious injuries. Police do not believe impairment was a factor. The driver is cooperating with the investigation.

“You can understand, he’s crushed,” Pennington said. “I don’t know about you, but I can’t even imagine losing my child in an accident like this.”

Maryland car crash kills 5 children, injures 2 adults

(CNN) — A Saturday morning crash killed five children and injured two adults, Maryland State Police troopers said.

Prince George’s County police and fire responded to reports of a car crash shortly before 5 a.m.

Police say a preliminary investigation shows that the 2005 Chrysler Pacifica went off the left side of the road into a wooded area, hitting several trees before spinning into a snow-covered field. Inside the car, police say, were two adults: 32-year-old Dominique R. Taylor, identified as the driver, and 23-year-old Cornell D. Simon in the front passenger seat.

Both were transported by Prince George’s County Fire Department medic units to UM Prince George’s Hospital Center and remain there while undergoing treatment for injuries.

In the back seat of the car were five children ranging in age from five to 15. All of the children were ejected from the car during the crash and were pronounced dead at the scene, Maryland State police said.

The early investigation has led police to believe the children were at the back of the car and were not properly restrained by seat belts at the time of impact.

Authorities say the cause of the crash is still under investigation.

Maryland Governor Larry Hogan tweeted on Saturday that he was “deeply saddened” by the news and that he was “praying for everyone involved.”

Utah teen narrowly escapes tragedy after rock smashes into her car in Price Canyon

CARBON COUNTY, Utah -- A teenager in Carbon County experienced a close call after a giant rock crashed through her windshield while she was driving, barely missing her.

Gabrielle Sainsbury said she has a massive headache following the crash and is really shaken up, but she feels lucky to be alive, knowing that the rock could have easily killed her.

Sunday just before sunset, she said she was driving through Price Canyon to her college dorm when the rock hit her.

"I couldn't believe that had happened, and if I wouldn't have swerved, it would have been me and I would have been crushed," Sainsbury said.

The last 24 hours, Sainsbury said she has relived the moment countless times.

"I keep seeing my windshield shatter in front of my eyes," she said. "It was very scary."

Sainsbury said she passed out, and when she came to, multiple men got her to safety.

UHP responds to 410 crashes statewide following storm

SALT LAKE CITY — Utah Highway Patrol has responded to 410 crashes across Utah, according to a tweet Wednesday.

Over half of those crashes, 211 to be exact, occurred between midnight Wednesday and Wednesday afternoon.

An 11-car pileup near the Davis and Salt Lake County border closed southbound Interstate 15 for two hours Wednesday morning, from 10:30 a.m. to 12:30 p.m.

Today, Little Cottonwood Canyon has been closed due to avalanche danger.

Police identify pedestrian hit, killed by car in Sandy

SANDY, Utah — Sandy Police have identified the man who was hit and killed by a car while walking in a crosswalk Thursday afternoon.

Police identified the victim as Sandy resident, 44-year-old Fernando Cruz Lavana.

Sandy Police said Lavana was walking in a pedestrian crosswalk in the area of 10040 S Wasatch Blvd when he was struck by a white Toyota Camry that was heading southbound on Wasatch Blvd.

Lavana succumbed to his injuries and died on scene, according to Sandy PD.

Police said the driver, a 68-year-old from Sandy, is complying with police. Police also mentioned the driver did not appear to be impaired.

The area of the accident was cleared and able to be used as of 5:40 p.m. Thursday.

The accident is still under investigation, police said it could be several days before they know what caused the crash.

“Even though the cause of this accident has not been determined yet, I always look at tragic situations as a strong reminder to any pedestrian before they enter the roadway to take a moment to ensure there’s not an approaching hazard,” said Lt. Dean Carriger with Sandy PD. “Make sure the operator of a motor vehicle sees you and make sure they have the ability to stop if they see you before they enter into the roadway.”

Woman in critical condition following auto-pedestrian crash in Utah County

UTAH COUNTY — A woman is in critical condition following an auto-pedestrian crash in Utah County.

Utah Highway Patrol Troopers were dispatched to a crash on SR-147, otherwise known as 6400 South, at 6:24 p.m.

UHP described the area of the crash as a low-light area without streetlights and said evidence suggests the vehicle in the incident was traveling at the posted speed limit of 50 mph.

The female pedestrian, UHP said, crossed the street to put out her trash scan and walked back into the roadway, directly in the path of the vehicle. Troopers believe the woman may have partially impaired vision and said she was wearing dark clothing, both things which contributed to the crash.

She has been transported to the hospital in critical condition. The driver remained on scene and is cooperating with investigators, UHP said.

Lanes open on Bangerter Highway after semi rollover

SALT LAKE COUNTY — A crash on Bangerter Highway at SR-201 in Salt Lake County closed all northbound lanes Wednesday afternoon.

According to Utah Highway Patrol, a semi was exiting SR-201 eastbound, turning left onto Bangerter Highway northbound, traveling too fast when the truck and trailer rolled onto the concrete barrier that separates northbound and southbound traffic on Bangerter Highway.

No one was injured in the crash and there were no other vehicles involved.

However, it did take several hours for troopers to clean up the accident, as they had to offload and upright the trailer.

The semi-trailer was loaded with large paper rolls, each weighing up to 5,000 pounds.

A forklift had to be brought in to move the paper rolls to a new trailer.

Husband, family of pregnant woman killed in Highway 40 crash upset suspect no longer facing charges

WASATCH COUNTY, Utah — The husband and family of a pregnant Duchesne County woman killed in a crash are in disbelief over what will happen to the man accused of causing the crash.

The family said they learned Tyson Uresti will no longer face charges in the death of 22-year-old Shantell Young.

A pregnant Young and her husband, Macoy Young, were headed east on Highway 40 in Wasatch County in late October when authorities said Uresti swerved and hit them head-on.

Uresti was accused of escaping state custody and stealing the vehicle he was driving.

Shantell and her unborn baby died in the hospital. Macoy is still recovering from severe injuries.

He said he had to re-learn to walk, and now has to do everything left-handed because he can no longer use his right hand.

The 25-year-old said he's now hundreds of thousands of dollars in medical debt, that he'll have to pay for.

From his neck hangs his and Shantell's wedding rings on a chain.

"I keep it on me all the time," Young said. "It helps me to kind of feel her presence a little more."

They remind him of the memories and life he and Shantell shared together. The couple was happily married and expecting their first baby-- a boy.

Young said they had just held their gender reveal the week of the crash.

"It's what every father dreams for, is to have a son," he said. "My life, it was perfect. Up until that day."

Young said he can remember the crash, and what happened right after it.

"Me and Shantell's car caught on fire, and [Uresti] didn't help. He ran and hid," Young recounted.

Not only would the family find out that Uresti allegedly absconded from state custody before the crash, but this apparently wasn't the first time.

According to court documents, Uresti was arrested for a similar absconding and vehicle stealing incident in May of 2018. Court documents stated that Uresti had run from the Utah State Developmental Center in American Fork, where he was staying due to a court order.

The center provides resources and supports people with disabilities.

Uresti faced a number of charges in the crash that killed Shantell and her unborn child.

This week, the court declared Uresti to be incompetent-- meaning the criminal case won't be moving forward.

"Those charges will not be on his record, like it never happened," said Heidi Brady, Shantell's mother. "That makes me really angry."

According to the competency ruling, the state will once again take custody of Uresti.

"He's going to be taken back to a facility in American Fork, that he's already escaped from twice," Brady said.

She said she hoped Uresti would be committed to the Utah State Hospital, and be placed on secure lock-down. However, she said she was told anyone deemed incompetent cannot be sent there.

For the family, this means they won't get justice and closure, and they said they are disappointed and terrified that Uresti is returning to the facility he is alleged to have escaped from.

In the competency ruling, the court urged the Department of Human Services and Division of Services for People with Disabilities, "to take all necessary precautions to ensure that Mr. Uresti will be unable to abscond and repeat the alleged behavior in this matter."

While the Department of Human Services could not comment on this specific case, they did issue this statement:

"Individuals with intellectual disabilities deemed by a court as not competent to assist in their own defense are not committed to a prison or lock-down facility. Private and public facilities exist to provide individualized behavioral and medical treatment programs within environmental controls like higher staffing ratios and delayed locks on doors, to reduce the risk of harm. Our primary aim in determining placement is always that of the well-being and safety of the individual, the staff who serve them, and the community."

A fundraising account has been set up to help the family, as Macoy continues to face mounting medical debt from the crash.

1 person dead, 4 LDS missionaries injured in car crash in New Caledonia

NEW CALEDONIA – One person is dead and four missionaries, including one from Utah, of the Church of Jesus Christ of Latter-day Saints were injured in a car crash Saturday.

A spokesman for the Church of Jesus Christ of Latter-day Saints issued a statement identifying the missionaries as Elder Jacob Hable from Jakarta, Indonesia; Elder Jeremie Champoux from Drummondville, Canada; Elder Gordon Kimball from Bluffdale, Utah; and Elder Atepa Temaiana from Mo’orea, French Polynesia.

The four missionaries are expected to make full recoveries.

According to the statement, there were three occupants in the second vehicle where one of the occupants passed away.

6-year-old girl in critical condition after being hit by car in South Salt Lake

SOUTH SALT LAKE –A 6-year-old girl is in critical condition after she was hit by a car in South Salt Lake Wednesday evening.

Executive Officer Gary Keller, South Salt Lake Police, said the crash occurred near 3575 South and West Temple.

Lt. Mike McNaughton with South Salt Lake Police said the girl was hit while in an alleyway.

"She was actually running from a garage area to a vehicle when she was struck," McNaughton said. "The other vehicle was actually coming between two parked vehicles in the alleyway."

The driver is cooperating, and police do not believe speed or impairment were factors in the crash.

This is the second case of a child being hit by a car in Utah on Wednesday. The other incident left a 6-year-old girl dead.

With warmer weather, police worry there could be similar accidents. Police said the crashes are a good reminder for drivers to be aware of their surroundings, and always check for children and other pedestrians.

Police identify teens killed in late night crash in Sanpete County

SANPETE COUNTY, Utah - Three high school students were killed in a car crash east of Mount Pleasant Friday night.

The Utah Highway Patrol said the crash occurred on Power Plant Road.

Troopers said a pickup truck was on Power Plant Road when a passenger car ran into the back of it at a high speed.

Three people in the car were killed. Police said the car caught on fire and a paramedic arrived quickly to remove everyone from the car before burning.

The three teenagers have been identified as 18-year-old Ryan Lyman of Ephraim; 16-year-old Julie Oldroyd of Fountain Green; and 16-year-old Kodi Wheeler of Fountain Green.

The driver of the car was the only one wearing a seatbelt. She survived and is home with her family.

Kodi Wheeler's younger sister, Emily, said she remembers driving with her sister on Power Plant Road.

“I'm not sure any of us have let it sink in yet," Emily Wheeler said. "It’s like riding a roller coaster almost where you’re like riding up up up up and then you reach the curve and you just go down. That’s what that road’s like.”

Emily described her sister as incredibly positive and happy. She said Kodi had been having a great week and loved spending time with her friends.

"She wrote five things about me every single day, and she ended it with a quote each time," Emily Wheeler said.

The speed limit on Power Plant Road is 35 miles per hour. Police said they were unsure how fast the car was going when it crashed into the pickup truck, but they believe it was over the speed limit.

"Just appears she was going a little bit too quick over this hill and couldn’t stop in time," UHP Lt. Clay Morgan said.

Kodi Wheeler's best friend, Julie Oldroyd, lived a few doors down.

Julie's brother, Wesley, said it's hard to find words to describe his sister.

"Always been the sweetest little girl that you'd ever meet," Wesley Oldroyd said.

Knowing that young people like to frequent Power Plant Road, Lt Morgan has this advice:

"Pay attention, slow down," he said.

Police will determine whether any charges will be filed when the investigation is complete.

FOX 13 has chosen not to identify the driver at this time.

Both the families of Julie Oldroyd and Kodi Wheeler said they don't blame the driver, they can't imagine what she'd going through, and they will continue to support her.

"You're my sister now too, and we're getting through this together," Emily Wheeler said.


Bike Accidents

Bike accidents can be classified as any accident that takes place involving one or more cyclist. Although bike accidents can occur in a wide range of circumstances, traffic-related bike accidents, such as those involving vehicles, are amongst the most common within the legal spectrum.

Types of Bike Accidents


A. A contact bike accident is one that results from direct contact between a bike and a vehicle, pedestrian, or other object. As the name suggests, an accident can only be classified as contact accident upon physical contact between two objects. An example of a contact bike accident is a vehicle striking a cyclist on a street.

B. A stationary, contact bike accident is result of a cyclist having impact with a stationary object. Some of the most common stationary, contact bike accidents occur as a result of the opening of a car door as the unsuspecting cyclist passes. This is known within the biking community as “Dooring”. Dooring can not only be dangerous for the cyclist, but can also injure the individual exiting a vehicle. Dooring can also result in damage of personal property.

C. A non-contact bike accident results from indirect contact between a bike and vehicle, pedestrian, or other object. Unlike a contact accident, a non-contact accident can be caused in an attempt to avoid an accident. An example of a non-contact bike accident is a vehicle unlawfully operating in a driving lane, which causes a cyclist to swerve and fall. Although there was no direct contact, the event has nonetheless resulted in an accident.

Bike Lanes

In many cities, there exist bike lanes, which are areas of roadways that are specifically designated for the operation of bikes and cyclists. Due to the fact that bike lanes exclude automotive vehicles, this allows for lessened opportunities for bike accidents to occur.

Identifying a Bike Lane


Although there are no nationwide, uniform designating symbols for the identification of bike lanes, they can be found on either side of the street. While some cities display a bicycle symbol in order to designate bike lanes, other cities illustrate bike lanes with alternate coloring.

Right of Way

The laws for safe cycling vary from state to state and country to country. Adherence to safe bicycling protocol is the most effective way to avoid bike accidents. In the busiest cities, cyclists must not only adhere to all vehicular traffic laws, but must also ride “with traffic”. This means that cyclists must operate their bikes in the same direction as the flow of traffic. However, in residential areas, cyclists may be permitted to ride “opposite traffic”.

Results of Bike Accidents

A. Bike accidents can result in both physical injury and pain and suffering.

B. Bike accidents can result in damaged property.

C. Upon sustaining damage or injury as a result of a bike accident, one is encouraged to consult with legal representation in order to investigate options of recourse and compensation. Regardless of the nature of the accident, compensation can be awarded without direct contact between a bike and another vehicle.

A boat accident can be classified by any accident in which one or more marine vessels is involved. Although boat accidents share many similarities to car accidents, such as damage and injury sustained as a result of collision(s), boat accidents possess an added element of danger due to the presence of water. Drowning accounts for upwards of 7,000 deaths per year. The added danger of drowning in tandem with physical injury, in addition to property damage, allow for boat accidents to be some of the most potentially dangerous accidents. However, with proper training, participation in safety courses, and certification, boat accidents can be avoided. 

Types of Boat Accidents


Boat accidents vary from large, commercial vessels to small, private boats. Furthermore, boating accidents can range from injuries sustained on cruise ships to those sustained while participating in water sports, such as water skiing and wakeboarding. 

While boat accidents can involve both damage and injury due to collision, the United States Civil Air Patrol unit reports that upwards of 70 percent of boat accident fatalities result from drowning. Other causes of boat accidents include improper and/or reckless operation.

DWI Boating Laws: Boating laws involving the improper operation of a boat while intoxicated mirror DUI laws in their respective place of operation. For example, in a state where the legal limit is .08 blood alcohol content (BAC) for the operation of a motor vehicle, the same applies for the legal operation of a boat. Furthermore, the penalties are mirrored, as well. Depending on the severity of the boat accident, penalties for DWI boat accidents can include fines, loss of boating licenses, and jail time. 

Certifications


Boat Operator’s Certificate/Boat License: The protocols for legal operation of boats vary from state to state. In certain states, obtaining a boat license must be achieved through the Marine Services Bureau, while other states require the acquisition of a boating license through the Department of Parks and Wildlife. Every state not only maintains a standard of legal age to operate a boat, but also a regiment of courses and examinations to earn a boating license. Boat accidents resulting from the improper or illegal operation of a boat can result in the loss, revocation, or suspension of a boating license. 

Boat Operators Insurance: Akin to vehicle insurance, the operation of a boat requires the owner/operator to possess insurance. Boating insurance not only covers the owner/operator of a boat, but also covers damage or injury sustained by, or as a result of, that specific owner/operator. The operation of a boat without insurance is both unlawful and illegal. 

Lifesaving Courses: As previously stated, over 70 percent of fatal boating accidents result from drowning. The completion of a lifesaving course such a lifeguard certification and/or CPR can prevent drowning resulting from boat accidents. 

Safe Operation

Life jackets and life vests should always be worn by every individual on the boat.

Alcohol – or any other mind-altering substance – should never be ingested while operating a boat.

In the event of damage or injury sustained as a result of a boat accident, individuals are encouraged to consult with legal professionals in order to evaluate the details of their respective cases. There exist attorneys who specialize in boat accidents.

Gymnastics Accidents

Gymnastics accidents are classified within the scope of athletic, or sports accidents, that can range in severity. Due to the wide range of gymnastic activities, there also exist a wide range of potential gymnastic accidents to which gymnasts may succumb. 

Danger Latent within Gymnastics

Without proper training – and in certain cases, even with proper training – gymnastic accidents can take place. Due to the elevated and aerial motions and maneuvers inherent within the sport itself, the chances of injuries are heightened. The rigorous expectations and maneuvers exuded by even the most trained gymnasts can result in gymnastic accidents if either the timing or placement of such exertions is not perfect. This is not to say that participation in gymnastics should be avoided. However, the strict safety precautions instituted by the United States Gymnastic Association (USGA) must be met in order to reduce the chances of gymnastics accidents. 

USGA Regulations

Although the parameters and specifications vary depending on the gymnasium in which gymnastic equipment exists, the USGA maintains strict requirements for the maintenance, operations, and upkeep of all professional-grade gymnastics equipment. Furthermore, only padding and protective gear that has been both endorsed and/or sponsored by the USGA is deemed acceptable for use in competitive gymnastics.

Type of Gymnastics Accidents

The Pommel horse is considered to be one of the primary and most recognizable pieces of gymnastics equipment. The Pommel horse consists of a padded bar that sits atop a metal stand with fitted handles. From top to floor, a Pommel horse measures 3.77 feet. Gymnastics accidents involving the Pommel horse have included a gymnast being ejected from the horse and impact between a gymnast’s body part and the horse. Without proper padding beneath the Pommel horse, a gymnast is liable to be injured due to slips and falls from the Pommel horse.

The parallel bars are amongst the most dangerous of all gymnastics equipment and are responsible for the majority of gymnastics accidents that take place. Parallel bars measure 6.6 feet from bar to floor and, as a result, slips and falls resulting from mistakes during routines can result in injury. Gymnastics accidents involving the parallel bars have included paralysis, broken bones, and brain injury. Due to their height, gymnasts without proper training or proper protection are encouraged to avoid participation in parallel bar routines. 

The rings are an example of elevated gymnastics equipment that can sit upwards of nine feet from floor to ring. Akin to the parallel bars, mistakes in gymnastics routines can lead to serious injury due to the height of the rings. 

Gymnastic Accident Precautions and Recourse


Regardless of the level of gymnastic activities, which can range from physical education in schools to a professional level, proper precautions must be taken. Not only must the equipment be in proper working order, but the protective measures must be maintained. Although there exists inherent risk in the participation of gymnastic activity, injuries sustained as a result of negligence should be explored and assessed by legal professionals specializing in such matters.

Although participation in athletic activities can be an enjoyable and recreational activity, safety requirements and regulations preventing accidents are two components to which one should always adhere. Types of Sports Accidents Sports accidents can range in severity. The severity most often mirrors the rigor latent in the athletic activity. Sports involving physical contact and large groups of people render a higher volume of sports accidents. However, there exists no formula to predict injury. Even spectators uninvolved in the athletic activity have been injured due to sports accidents. There exist three primary types of sports accidents. These do not address the nature of the injury, but rather the classification of the sports accidents: Sports accidents involving spectators, who are not participants in the sporting event in question, have occurred in a variety of circumstances. Ranging from stray equipment ejected into the stands to flaws in the integrity of the building’s structure (falling lights, collapsed bleachers, etc.). Sports accidents involving spectators should be investigated further in the event that injuries have been sustained. Five Eastern States will Participate in Earthquake Drill

 

The Federal Emergency Management Agency (FEMA) announces that people in five states and the District of Columbia will participate in the first earthquake drill for the eastern states on October 18, 2012 at 10:18 a.m.  The drill is being called the Great SouthEast ShakeOut.
 
Georgia, South Carolina, North Carolina, Virginia, Maryland, and the District of Columbia want all schools, businesses, organizations, government agencies, towns, and households to participate in the first-ever drill.  The drill is being led by FEMA, the Central U.S. Earthquake Consortium, and other partners as well.  
 
A similar drill occurred in the nine Central U.S. states in February of 2012, and about 2.4 million people joined in.  
 
During the drill, people are encouraged to DROP to the ground, take COVER under a sturdy object, and HOLD ON until the earthquake stops.  The common action during an earthquake emergency is known as “Drop, Cover, and Hold On.”  
 
The east-coast drill is being performed because of the M5.9 Mineral, Virginia earthquake that occurred on August 23, 2011.  Earthquakes are rare on the east coast, but the earthquake caused a considerable amount of damage to Louisa County in Virginia and Washington D.C.  
 
Robert Welch, the FEMA Region III Acting Regional Administrator wants as many people and organizations to join and register for the event as possible.  Welch stated, “We were reminded last year that earthquakes can and do happen anywhere.  Everyone should make plans and take actions to ensure that disasters do not become catastrophes and this drill is an excellent start.” 
 
FEMA states that about 10.5 million people are expected to participate in the drills on October 18.  If FEMA is right, the drill will be the largest safety drill ever held at once.
